Employee's fingertip amputated in packaging machine

Employee #1, a packaging machine operator, was operating a machine along a line that filled small packets with a powdered product. A small hopper above the machine fed the powder, and a belt-driven agitator kept it flowing. A lead person had removed the guard and then left to request assistance from maintenance. While she was gone, Employee #1 pressed against the belt to make the agitator move. Her finger became caught between the belt and shaft, amputating it near the first knuckle.
