Investigation Summary
Employee #1, a door technician, and a coworker, had just repaired a 16 ft roll-up door and were reinstalling it. While Employee #1 was on a ladder, the reversing device activated and his right hand became caught in the motor chain drive gear. Two of his fingers were amputated at the first knuckle.
