Investigation Summary
Employee #1, a painter, was working on a scaffold that had been set on a planked floor. As he was descending from the scaffold, he either slipped or jumped and landed on the planked floor. He then fell approximately 13 ft through a 12 to 14 in. open space between the floor and a wall, landing on a concrete floor. He suffered a fractured skull and collarbone.
