Employee's fingertip amputated in mechanical punch press

Employee #1, a machine operator, was using a mechanical punch press to trim scrap from a small metal part. He activated the press, which required both hands, but then reached with his right hand into the point of operation before the slide had completely descended. The slide amputated the tip of his right middle finger.
