Employee's hand crushed in press

Employee #1 was operating a 60-ton Federal press. His left hand was on the feed table when the table moved backward toward the point of operation, activating the machine to cycle. The press came down, crushing Employee #1's hand. This accident was the result of a press malfunction. The push/pull pin that controlled the feed table had become misaligned, allowing the table to freewheel.
