Investigation Summary
Employee #1 was placing a new buffing wheel on a buffer. He was tightening the locking nut on the threaded shaft, and turned the switch to spin the shaft to quickly thread the nut on. A portion of his glove became caught between the tightened nut and the rapidly spinning buffing wheel, tearing off his finger. The employer chrome plates automobile wheels, and Employee #1 was performing his normal and customary duties.
