Employee's fingers amputated in sliding table saw

Employee #1, age 19, was using an Italy sliding table saw, model #6R16610, serial #101058 to cut a board. He was operating the saw with his right hand and was reaching across it with his left hand when it contacted the saw blade. Employee #1's small finger, ring finger, and part of his index finger were amputated. Although the saw was equipped with a Bleseyer safety guard, it was in the 'up' position, leaving the blade exposed.
