Investigation Summary
Employee #1, a factory worker, was using a 12 in. table saw to cut a small piece of plastic. The saw had a jig set on one side of its blade, and the piece of plastic had been placed in another jig. He encountered an obstruction while cutting and moved one side of the jig away from the blade. The saw was still running and as he attempted to reposition the jig with the plastic his hand slipped and contacted the saw blade. His right middle finger and part of his right ring finger were amputated. He also suffered multiple lacerations.
