Investigation Summary
At about 1:45 p.m. on April 04, 2003, Employee #1, a carpenter of the Guaranty Management Corporation, in Chino, California, and a coworker, were working on the construction of a small maintenance shed. Employee #1 measured and marked on the 2 ft by 6 ft wooden facia board while the coworker was holding the wooden piece. After marking the facia board to the required size, Employee #1 was climbing down from a Werner, model Number 6200, 6-ft fiberglass stepladder. He missed a step and fell backward, landing on his head. Employee #1 sustained head trauma injuries. He was taken to Chino Valley Medical Center, Chino, California, where he was pronounced dead.
