Employee's arm amputated when caught between melt and coil

On December 18, 1998, Employee #1 and coworkers were replacing a high-tension 1-in. steel eye band that had broken off a melt coil. The coil was suspended on the C-hook, 5 in. to 6 in. off the ground. The remaining bands broke, releasing stored compressed energy and pinning Employee #1's arm between the melt and a stationary steel coil. Employee #1 sustained a severe crushing injury to his left arm and was transported to the Loma Linda University Medical Center where he was diagnosed and surgically treated for a total avulsion of the left forearm, elbow and upper arm. The employer had a written, implemented procedure for banding and lifting the coils and all employees in the department have been trained in the operation. The coils were to be set on the ground during banding.
