Investigation Summary
On May 4, 1999, Employee #1 fractured his arm and face when he fell 20 feet onto a cement floor. The fractures required re-constructive orthopedic and plastic surgeries. Employee #1 was on a beam holding a purlin down while a co-worker welded the purlin to clip onto the main beam. As he was holding the purlin down, he fell 20 feet onto the cement floor. The height of the steel framed structure was 44 feet. Employee #1 did not use any fall protection. He was hospitalized for eight days at the Arrowhead Regional Medical Center and is still undergoing treatment. His employer failed to obtain a permit from the Division and failed to provide fall protection for Employee #1.
