Investigation Summary
At approximately 7:20 a.m. on April 15, 1999, Employee #1, a "leadman" in the hot-strip mill, was walking on top of stored coils in the hot-strip-finishing department. Employee #1 was looking for coils that were improperly marked on the prior shift. Employee #1 did not work in the hot-strip-finishing department, he was not wearing gloves, and the soles of his boots were greasy. In addition, the hot surfaces of the pipes had no thermal insulating materials nor were they otherwise guarded against contact and there was no safe access way to the coils. Employee #1 sustained serious burns to his hands, upper torso, and both legs when he stepped from a cool coil to what he thought was another cool coil. The coil Employee #1 stepped on was actually hot (575 degrees). Employee #1 slipped and fell between the hot coil and a cool coil. Employee #1 was transported to the hospital where he was treated for second-and-third-degree burns. Employee #1 was hospitalized 6 days.
