Investigation Summary

Investigation Nr: 201151990
Event: 10/28/1998
Employee Amputates Finger in Machine

On October 28, 1998, an employee was working with a Slitter Operator. A Slitter Operator, working at a raised, main control panel, was responsible for controlling the movement of the metal strip during processing that ran a coil to completion. The employee picked up a mult to bend it. The employee was holding the mult, when the Slitter Operator jogged the mandrel. The employee's finger was caught by an over arm divider blade. The employee received medical attention; however, hospitalization was not required. The Herr-Voss tension leveler was equipped with a machine power control for each employee; however, the controls were not interlocked to prevent operation, unless all controls were operated simultaneously.
