EMPLOYEE SUFFERS FINGER AMPUTATION

Around 6:00 a.m. on April 3, 1998, Employee #1, a supplemental operations contractor working as a general laborer, suffered a single joint amputation of the left index finger. Employee #1 was preparing to clean out a centrifuge. He stood on a platform and opened the door, causing a safety switch to shut down the motor. However, the basket was still rotating. Employee #1 turned to step down off of the platform, he slipped and his left arm entered the basket area. His arm was caught by the force of the basket and the gutter cleaner's movement. Employee #1 was transported to the hospital for treatment. Employee #1 had not been properly trained to operate the centrifuge and the company did not have a written and implemented procedure in place for training subcontractors.
