Investigation Summary
At approximately 11:30 p.m. on March 26, 1998, Employee #1 was standing on the extended tray at the back of a flatbed truck while Employee #2 was driving in reverse. Employee #1 was picking up traffic cones and stacking them on the bed, which is common practice in the industry, when the truck was struck by a pickup truck driven by a drunk driver and was killed. At the time of impact, Employee #2's head struck the windshield. Employee #2 was taken to the hospital, treated, and released.
