Employee killed when struck by drive shaft part

At approximately 11:30 a.m. on November 17, 1997, Employee #1, a tow truck operator, was disconnecting the drive shaft from the truck's differential unit when a section of the drive shaft was ejected from the universal joint and struck him in the head. He was transported to Kaiser Hospital in Fontana, where he was pronounced dead. Employee #1 had not released the brakes, which resulted in the torque loading of the four-section drive shaft.
