Investigation Summary
At approximately 6:45 p.m. on July 7, 1997, Employee #1, age 19, was on the fifth step of an 8 ft stepladder, retrieving a box from an approximately 81 in. high shelf. The box slipped in his grasp and he lost his balance, falling approximately 60 in. onto the concrete floor. Employee #1 suffered a serious ankle injury and was transported by paramedics to San Antonio Community Hospital, where he remained for three days.
