Investigation Summary
At 4:55 a.m. on September 27, 1997, Employee #1 fractured his right femur when the brakes of the forklift, which he was driving backwards into Aisle 49RS, did not respond as expected. The forklift hit a rack, throwing Employee #1 into the same rack and then pinned him against the rack. Employee #1 was hospitalized for more than one week.
