Investigation Summary
At 4:25 a.m. on January 16, 1998, Employee #1 was seriously injured when the forklift he was operating spun and slid out of control. Employee #1 was ejected when the lift truck hit the slat bed saw, rebounded and then struck Employee #1 in the lower torso, crushing him against the saw.
