Investigation Summary
At approximately 12:00 noon on August 28, 1995, Employee #1 was using an unguarded table saw to cut a 2 ft by 6 in. piece of wood. The blade caught the wood, causing it to kick back. When Employee #1 attempted to catch the wood he placed his right hand on the unguarded blade. He sustained an amputation of his right thumb and a severe cut to his right index and middle finger. He was hospitalized for two days.
