Employee's hand crushed in nip point of conveyor

At approximately 2:00 a.m. on February 23, 1995, Employee #1, of 3M Company, was removing a box from a conveyor when her left hand became caught between the conveyor belt and the pop-up roller. Her hand was crushed. The roller that was designed to pop out did not. The opening between the conveyor belt and pop out roller was less than a 1/4 inch. No citation was issued.
