Investigation Summary

Investigation Nr: 201158730
Event: 12/08/2003
Employee is injured when crushed between vehicles

At approximately 7:30 a.m. on December 8, 2003, Employee #1 and two coworkers were collecting air cargo containers and dollies that had been left in Lot C of the Ontario International Airport. The two coworkers were operating tugs while Employee #1 was preparing a dolly with a mounted air container for coupling to a tug that was operated by one of the coworkers. Employee #1 had released the brakes on the container dolly by lowering the dolly tongue to a level where it could be coupled to the hitch on the back of the tug. When the coworker attempted to back the tug into position, he lost control of the vehicle and it accelerated rapidly toward the dolly and pinned Employee #1 between the tug and the front of dolly frame. Employee #1 suffered a compound fracture of his lower left leg and was hospitalized at Arrowhead Regional Medical Center in Colton, CA for 5 days.
