Investigation Summary
On November 12, 2003, Employee #1 straighten a bend in a stainless steel buck plate that was just above the vertical supports for the ICC step. He used the hand-cranked winch with chains attached to the trailer's king pin at the front and a hole in the metal plate the had welded to the buck plate at the rear of the trailer in an attempt to also pull the buck plate back into proper position. When he was hand cranking the winch, the strain on the weld became too great and the metal plate released from the back of the buck plate and flew along the path of the chain. The metal plate struck Employee #1 causing a broken nose, facial lacerations and significant trauma to his right eye. He was transported to a hospital and was treated for a broken nose, facial lacerations and eventually, a permanent loss of sight in one eye.
