Employee's fingers amputated by cut off saw

At approximately 10:30 p.m. on March 2, 2004, an employee was cutting a 7-in. long by 1.5-in diameter plastic flex pipe with a CTD, Model Number F255, and Serial Number X1014 cut-off saw. The employee used a two-hand control device to activate the saw. During picking of the cut pipe, the upward retrieval of the saw blade suddenly became sluggish at which time the running saw blade caught and pulled the employee's gloved right hand causing amputation of his middle, ring and little fingers. Initially, the employee was taken to the Arrowhead Regional Medical Center, Colton, CA. for treatment. Then he was taken to UCLA medical Center for surgery to attach his amputated fingers. The employee was hospitalized for 2 days.
