Investigation Summary
At approximately 2:30 p.m., on October 26, 2004, Employee #1 sustained serious injuries to the back of his right knee and fracture of his right femur, when his leg was pinned between the rear bumpers of his 2002 Nissan Xterra SE Sport Utility Vehicle (SUV) and a 1979 White Chevrolet (License # 5C47763) fuel truck. Employee #1 was putting his tools into the tool box in the rear hatch of his SUV, while standing at the right rear corner of his SUV, having his back toward the backing up fuel truck, driven by a helicopter pilot at the concrete ramp of the Rancho Cucamonga Fire Department Station #174. The Chevrolet, while backing up, pinned his right leg against the rear bumper of his SUV causing serious injuries to his right leg. He was hospitalized for the injuries.
