Employee Dies after Being Pinned by Forklift Load

At approximately 9:45 a.m. on September 13, 2004, Employee #1 was unloading two 6,000-pound bundles of lumber from a flatbed truck with a Caterpillar forklift, Model Number DP50K. He stopped the forklift, with the load raised, next to two bundles that had been previously placed in the storage area. He placed three 2-inch by 4-inch spacers on top of the existing 50-inch-high stack. While he was in front of the forklift, the forklift rolled forward and pinned him between the existing stack and the raised load. He sustained blunt closed head trauma, pulmonary contusions, and blunt abdominal trauma injures and was transported to Arrowhead Regional Medical Center in Colton, CA, for treatment. He died two days later on September 15, 2004.
