Investigation Summary
At approximately 7:00 p.m. on July 23, 2004, Employee #1, a furniture carpenter, was making cuts on small blocks from a 2-foot long board using a Powermatic Band Saw Number 34, Model Number 81, Serial Number 781150. Employee #1 was making a curved cut when the small block stuck on a hole in the bed, his left hand struck the running band saw blade amputating his left index, middle, and ring fingers. The blocks of wood were 2.75 in. by 2.75 in. by 1.25 in. thick. Employee #1 was hospitalized.
