Employee's hand caught in wheel grinder

At approximately 10:35 a.m. on May 11, 3005, Employee #1 was grinding a 4-foot long by 3/4-inch diameter steel bar on a Baldor pedestal grinder; model number CHT1216, serial number 773. The grinder was located next to an aluminum furnace at the forging facility. Employee #1 was holding the bar and grinding one end of it when the revolving grinding wheel pulled his gloved left hand, amputating his index and middle fingers. Employee #1 was hospitalized for surgery, treated, and released.
