Investigation Summary
At approximately 9:40 a.m. on December 18, 2004, Employee #1, an employee of a framing contractor for the residential construction business, was unloading a 2-ft. by 6-ft. pieces of lumber on top of a roof. Employee #1 and his coworkers were sheathing a two-story house. Employee #1 was crossing over the ridge near the gable end of the house when he accidentally fell approximately 20 ft. Employee #1 was killed. Employee #1 was not protected from falling hazards, by scaffolding, guardrails, or a personal fall protection system.
