Investigation Summary
On December 28, 2005, Employee #1 and #2 were working in a creamery. They attempted to remove the sensor gauge and its diaphragm from the bottom of a sour cream tank while the tank's agitator blades were rotating. When their fingers contacted the rotating blades, Employee #1 amputated his left middle finger up to the first knuckle, whereas Employee #2 lacerated his right index finger. Both employees were transported to a medical center where they received medical attention and were hospitalized.
