Investigation Summary
At or about 10:00 a.m. on July 1, 2005, Employee #1, a concrete laborer, was working on a light trade wooden scaffold equipped with a single railing at 30-inches or less above the work platform at the open side and ends. As Employee #1 was walking on the scaffold platform, he grabbed a wooden hand-hold support installed on the second story wall. The wooden support became loose and came off the wall and Employee #1 fell off the scaffold to the ground 15 feet below, sustaining a serious injury of his lower back, fracture of his right wrist and laceration on his left hand. Employee #1 was hospitalized.
