Investigation Summary
At approximately 11:50 p.m. on November 21, 2005, Employee #1 was operating a Raymond forklift in the northern part of the north dock of a refrigerated warehouse. He was learning to drive the Raymond forklift, Number 344, Model Number R35-C35TT, a split sheet forklift, in a rectangular pattern. He tried to steer it in a forward direction towards his right side, when it began to move towards his left side. He panicked, lost control and crashed into a bollard, 4-ft high by 6.75-in. in diameter. His foot was caught between the bollard and the forklift frame. He sustained a crushed left foot and was hospitalized.
