Employee Sustains Fractured Ankle in Forklift Crash

At about 9:00 a.m. on November 14, 2005, Employee #1 was operating a stand in forklift (brand Raymond and model number 31IS-R40TT) at an inclined ramp of the employer's distribution center, located on the SW part of the warehouse. Employee #1 was driving the forklift, with fully extended forks, in a backward direction down the inclined ramp to deliver a wooden pallet containing merchandise. A witness to the accident said that the pallet was dragging and Employee #1 attempted to raise the pallet up. At this time, the forklift crashed into the ramp wall and Employee #1's left lower leg got pinned between the forklift frame and the ramp wall, crushing his left foot and fracturing his left ankle. He was taken to Arrowhead Regional Medical Center, in Colton, CA, where he underwent surgery and remained for 3 or 4 days.
