Employee Injures Arm While Installing Diaphragm Press Machin

At or about 6:15 a.m. on July 28, 2005, Employee #1 was installing a diaphragm in the press bell of a linen washing press machine, located in the washroom of the facility. The employer primarily offers complete laundering, servicing, delivery and distribution systems to replace uniforms for employees of commercial outfits (i.e. hospitals, retail shops manufacturing & fabrication facilities). According to Employee #1, he along with a coworker were replacing a defective 43 ft diameter diaphragm of the bell section of a "MILNOR, Model Number MP5031A" linen washing press. During the installation of the diaphragm and ring assembly, Employee #1 was trying to align/center it. Suddenly, the bell (weighing 600 lbs) dropped and fell. The falling bell hit both employees, causing injuries. Employee #1 sustained serious left forearm injuries and he suffered non-serious bruising injury of his right forearm. Employee #1 was taken to Loma Linda University Medical Center, Loma Linda, CA for treatment. Surgery was performed, and Employee #1 was hospitalized for more than 4 days.
