Investigation Summary
On 16 August 2006, Employee #1, a new full-time worker with less than 30 days of experience, was operating a 14 in. Delta-Rockwell table saw. The employee did not turn off the saw and let the blade come to a complete stop, when he started cleaning wood dust and debris from the saw table. Although the blade guard was in place, the employee's left thumb went into the point of operation and was amputated. The employer asserted that table saw safety training was provided to the employee but there was no documentation to verify that training was delivered.
