Investigation Summary
On September 17, 2005, Employee #1 was climbing an extension ladder to repair the netting at the top of a 19-ft high fence. One of the rung locks broke off of the ladder causing it to collapse and leave Employee #1 hanging from the top bar. Employee #1 lost his grip and fell 20 ft to the ground. He fractured both wrists and sustained other impact injuries requiring six days of hospitalization. This was the first time when Employee #1 used an extension ladder at work, and he was not trained in extension ladder safety. A pre-use inspection or service check was not performed to ensure the ladder was in serviceable condition before it was used.
