Investigation Summary
At 9:15 a.m. on October 14, 2006, Employee #1, the operator, was servicing and adjusting the head pulley of a feeder belt at the asphalt production plant. Employee #1 slipped and fell on the moving gathering conveyor, which was located below a feeder belt. The feeder belt carried Employee #1 downstream to a transfer point, where the gathering conveyor dumped the aggregate onto the long conveyor belt. Then Employee #1 fell to the ground next to the unguarded self-cleaning tail pulley of the long conveyor. Employee #1's right arm was pulled into the unguarded pinch point between the skirt board and the long conveyor belt, which was located some inches away from the unguarded self-cleaning tail pulley. The gathering and long conveyors run at the speed of approximately 330 ft per minute. The long conveyor belt was 36-in. wide and its self-cleaning tail pulley had 12 in. in diameter, and was 38-in. long. The 10 cleaning fins were located 21 in. off the ground. Unable to free himself, Employee #1 was being struck continuously by the fins on the self-cleaning tail pulley. A coworker, who suspected that Employee #1 might be in trouble, proceeded to the accident location. With the aide of Employee #1, he located an equipment emergency stop mean and stopped the feeder belt. Employee #1 was air lifted to Loma Linda University Medical Center, where he was hospitalized for two weeks for the open wounds to his back, abrasion to his right arm, rib fracture, and other injuries.
