Investigation Summary
At approximately 1:00 p.m. on February 10, 2006, Employee #1 was ripping a work piece that was 12-ft by 5-inch by 0.75-inch using a table saw, a DeWALT, DW746, Serial Number 16213, 1.75-HP, 3,000 rpm, with a 10-inch blade. He was taking 0.75-inch off the 5-inch side. The saw blade was not guarded by the hood guard. It was a common practice to remove the hood guard assembly supplied with the saw from the manufacturer while performing this type of cut on this material to avoid damaging the wood with the pawl's fingers. The hood guard assembly includes the spreader and the anti-kickback pawls along with the hood guard. At the time of the accident, the blade projected at least 0.5-inch above the material being cut. The rip fence was not parallel to the line of the saw blade. In the middle of the cut, while Employee #1's left hand was placed on the work piece against the table, and his right hand was holding the tail end of the work piece, the work piece lifted up. Employee #1 pushed the work piece back down with his left hand into the unguarded blade, and his left index finger was amputated. Employee #1 was transported to John F Kennedy Memorial Hospital, where he was admitted for treatment.
