Investigation Summary
At approximately 5:00 p.m. on November 12, 2007, Employee #1 was using a chair fitted for a preschooler to climb up and stand on a table fitted for preschooler, so she could attach mobile art work on the ceiling of the "Squirrel Room" one of several classrooms located at the preschool. The distance is approximately 21.5 in. from the top of the preschooler table to the floor. The preschooler table was also missing one leg on one side of the table. Employee #1 stated she was unaware that the table was missing a leg. Employee #1 stated that the table tipped over while she was attempting to hang art work to the ceiling. Employee #1 stated her leg went to her head and then she heard her leg crack on the left side. Employee #1 called for help to a coworker located in the "Beaver Room" directly across from her room. The coworker found Employee #1 on the floor lying on her right side. Employee #1 sustained a spiral type fractures to her left tibia and fibula. Employee #1 was transported by ambulance to the hospital. Employee #1 was in the hospital for three days and had surgery the day after the accident occurred.
