Investigation Summary
On March 16, 2009, Employee #1, a carpenter, was working with a general building contractor performing remodeling work. Employee #1 was on a rolling scaffold platform 6 ft high and a coworker was on the floor level. The rolling scaffold was a Gemini utility scaffold by Bill-Jax, Part Number 0202-0510. The bottom base size of the scaffold was 74-in. by 29-in. The top rail height of the platform was 43-in. and there was a mid rail. There were 4 wheels-casters at the bottom which could be locked. There were provisions for extra attachments, outriggers on each side. With 1 outrigger at each side, a total of 2, the total number of wheels-casters was 8. Employee #1 was hanging a plywood wall using a screw gun. He lost his balance and fell on the floor along with the scaffold. He sustained serious head and brain injuries and was hospitalized.
