Employee Loses Thumb When Cut by Saw Blade

At 3:30 p.m. on January 31, 2014, Employee #1 was operating a table saw cutting a sheet of plywood (18 inches wide, 8 feet long and 0.5 inch thick). The table saw, Serial Number 04040660845, Model Number 66-TA SAW, Stock Number 2688114, did not have a hood guard covering the saw blade. A small piece of wood jumped over the saw blade, and Employee #1 attempted to remove it from the blade. His hand came in contact with the saw blade while it was still in motion. He was taken to Downey Medical Center then transferred to USC Medical Center, where he was hospitalized for four days. Employee #1 had his first knuckle on his left thumb amputated at the first knuckle, and he was treated for a deep laceration to his left index finger and palm.
