Investigation Summary
At approximately 12:30 p.m. on November 28, 2012, Employee #1, a 55-year-old female with Ducommun Labarge Technologies Inc., was performing some tests for the LPL lighting in the test room. The test unit-box was a black wooden box that measured 30 in. wide, 30 in. long and 36 in. high. There were two parts of the unit: the base was 12 in. high from the bottom, and the lid was attached to the box from the back with the hinge to allow the lid open vertically. Two locking hinges were installed on the sides of the lid to keep it in up position when the lid was fully open. For extra support, a wood stick (2.75 in. wide, 33.25 in. long and 0.5 in. thick) was used to keep the lid stay in up position. There were groves at each end of the wood stick that fit tightly with the edge of the lid and base. According to Employee #1 she attempted to take parts out of the unit, she lifted the lid up and it fell on her head before she could put a wood stick in place to prop the lid open. Employee #1 was taken to the Torrance Memorial Hospital on November 28, 2012 and was released from the hospital after few hours. Employee #1 suffered a laceration on her head that required six stitches. Employee #1 was later admitted to the hospital on December 2, 2012 for observation with medication reaction symptoms and was released on December 3, 2012.
