Investigation Summary
At approximately 1:00 p.m. on July 8, 2005, Employee #1, a sales engineer working for a solar panel installation company, was using a compact folding ladder to access the roof of a house. As he was getting from the ladder to the roof, the ladder slipped from beneath him. He fell 6 feet to the ground and landed on the ladder. He fractured his right hip and was hospitalized for 5 days.
