Investigation Summary
At approximately 12:30 p.m. on October 16, 1998, an employee was grading a contaminated dirt pile using a front end loader which was missing the steps used to egress the cab. During his lunch break, the employee attempted to egress the front end loader, grabbing hold of the handles on both sides of the cab and attempting to put his feet on the front end loader's structure. He slipped, lost his grip on the handles, and fell onto the concrete floor. He suffered multiple fractures to his right knee and was transported to Doctor's Hospital, Pinole, CA.
