Investigation Summary
At approximately 1:30 a.m. on September 10, 1998, Employee #1 was engaged in a roofing operation. The roof had an approximate 7 by 12 slope. The employer only had enough safety equipment available for one employee to be tied off. Employee #1 was doing roofing work when he slipped on some glue and fell approximately 15 feet to the ground. The employee was transported to John Muir Hospital, where he was hospitalized. Employee #1 sustained a fractured back and a chipped bone in his left wrist.
