Investigation Summary
At approximately 8:00 p.m. on April 17, 1998, Employee #1 was moving products in the loading area near door #73, preparing part of a shipment of dry goods to go to store #380. He stopped his transporter, stepped off it and began to move some of his load onto another pallet. As Employee #1 was working, he was struck by a coworker operating a CAT forklift, model F35, serial #FB2098. Employee #1 suffered a fractured left ankle and foot, for which he was hospitalized.
