Employee injured when fingers amputated by mixer gate

Employee #1, a laborer, sustained an amputation and crushing injuries to three fingers of his left hand while performing cleaning activities inside a Besser mixer used for manufacturing concrete blocks. The fingers could not be reattached due to the severity of the injuries. Employee #1 sustained his injuries when the mixer's large metal gate was closed on his fingers by his foreman, who had activated the air-actuated gate from the control room. Employee #1 was hospitalized for treatment of his injuries.
