COOK SLIPS AND FALLS ON WET SURFACE AND INJURES BACK

Employee #1, a catering cook, slipped in a puddle of water and fell while moving between the main kitchen area and the catering cook area through the adjoining walkway. After the fall, a coworker placed a "wet floor" caution sign near the area. Employee #1 was originally diagnosed with "acute lumbar strain", a non-reportable injury. Several months later, she was admitted to a regional medical center for back surgery. Her employer reported the accident to the Division of Occupational Safety and Health.
