Investigation Summary
At 7:00 a.m. on February 22, 1996, Employee #1, a forklift operator, was moving from a elevated pallet on the forks to the operator's position when he apparently pushed back the tilt lever for the mast. He became pinned and was crushed between the mast and the driver's cage. Employee #1 was killed.
