Employee's finger amputated in conveyor auger

At approximately 2:30 p.m. on October 21, 1999, Employee #1 and a coworker were engaged in routine maintenance on a rice dryer, greasing the shafts and bearings near the top of the elevator. While attaching the grease gun fitting onto the greasing nipple, Employee #1's shirt cuff became entangled in the chain and sprocket driving the conveyor auger. As he tried to pull his shirt free, his finger became caught in the nip point and was amputated at the first joint. He was driven to Sutter-Roseville Medical Center in Roseville, CA, and then transferred to Stanford Medical Center in Palo Alto, CA, where he underwent surgery to reattach his finger. The auger's guard had no back side, which allowed access to the nip point.
